目的 研究胃癌表达胃泌素受体(gastrin receptor,GR)的特征及意义。 方法 应用受体的放射配基结合分析法测定34例胃癌标本GR含量及其亲和力,并比较胃癌表达的GR与其部位、分期、分级、胃癌细胞DNA含量、邻近正常胃粘膜的GR及患者生存期的关系。 结果 34例胃癌组织中,16例(47.1%)胃癌GR阳性,其中14例(41.2%)表达高亲和力GR,2例表达低亲和力GR;在高亲和力GR中,9例表达高含量GR(平均含量39.5±14.4 fmol/mg蛋白),5例表达低含量GR(平均含量6.0±2.8 fmol/mg蛋白)。胃体癌(77.8%)和贲门癌(50%)较胃窦癌(20.1%)更易于表达高亲和力GR。相同部位的胃癌及邻近正常粘膜表达GR是一致的,但胃癌表达的胃泌素受体含量(39.5±14.4 fmol/mg蛋白)高于粘膜(26.1±16.6 fmol/mg蛋白)。Ⅲ、Ⅳ期胃癌表达高亲和力GR的比例(13/24)明显高于Ⅰ、Ⅱ期胃癌(1/10)。胃癌表达的GR与癌细胞的分级无关。随访23~61月发现,对于Ⅲ、Ⅳ期胃癌患者,13例表达高亲和力GR的胃癌,有11例死亡;而11例低亲和力GR或GR阴性的胃癌,仅有4例死亡。 结论 GR在胃癌自分泌生长过程中也是一个重要的因素;GR有利于预测患者的预后及指导GR拮抗剂的应用。
Objective To study the characteristics and significance of gastrin receptor (GR) expression in gastric cancer. Methods Receptor radioligand binding assay was used to determine the GR content and its affinity of 34 gastric cancer specimens. The expression of GR and its site, staging, classification, DNA content of gastric cancer cells, GR of adjacent normal gastric mucosa and survival time of patients were compared. Relationship. Results In 34 cases of gastric cancer, 16 cases (47.1%) were positive for GR in gastric cancer, of which 14 cases (41.2%) expressed high-affinity GR and 2 cases expressed low-affinity GR. In high-affinity GR, 9 cases expressed high-content GR (average The content was 39.5±14.4 fmol/mg protein), and 5 cases expressed low content of GR (mean 6.0±2.8 fmol/mg protein). Gastric cancer (77.8%) and cardiac cancer (50%) were more likely to express high-affinity GR than antral cancer (20.1%). The expression of GR was similar in the same site of gastric cancer and adjacent normal mucosa, but the gastrin receptor content (39.5±14.4 fmol/mg protein) expressed in gastric cancer was higher than that of mucosa (26.1±16.6 fmol/mg protein). The proportion of high affinity GR in stage III and IV gastric cancer (13/24) was significantly higher than that in stage I and II gastric cancer (1/10). The GR expressed by gastric cancer is not related to the grade of cancer cells. During the follow-up of 23 to 61 months, 13 cases of high-affinity GR expressing gastric cancer and 11 cases of low-affinity GR or GR-negative gastric cancer had only 4 deaths in patients with stage III and IV gastric cancer. Conclusion GR is also an important factor in the autocrine growth of gastric cancer; GR is helpful to predict the prognosis of patients and guide the application of GR antagonists.
