产前的超声检查可提示先天性肺肿块自然经过的基本情况,先天襄性腺瘤样畸形(CCAM)及其它先天性病变肿块行为的自发改善和纵膈转移情况虽已有报道.但其影响因素罕有所知.肺先天性肿块在子宫内生长与组织学诊断之间的关系尚无系统研究. 作者在该研究中检测了相同病理诊断的肿块在子宫内的生长形态是否一致;研究了胎儿胸径增大与肿块缩小在肿块行为改善和纵膈转移中的相关重要性;阐述了既有Ⅱ型先天性囊性腺瘤样畸形山有肺叶内分膈(CCAM/ILS)肿块的超声特证及生长型.材料与方法:观察了经产后手术、病理诊断及临床过程的8例先天性肺肿块胎儿的产前超声图像.采用定肿块横断面积合评价肺肿块的生长.选择以最能提供肿块的胸腔横位为准进行动态比较计算肿块横断而积和胎几胸腔横断面积及肿块胸腔比例. 结果:每例胎儿至少两次以上超声检查.首次发现肿块最早在孕期17周,最晚在29周(平均23周).手术切除经病理诊断为CCAM一Ⅱ/ILS者4例,其中3例肿块内大有1个以上囊肿超声图象,另1例只有肿块而无襄性超声图型.所有肿块均位于左下肺叶、术中都见肿块至少有一支动脉血管、而孕期超声检查均未发现肿块内有血管;CCANI-Ⅱ者2例、只有肿块图像而无囊性超声图

The lumps were found in the first 17 weeks of pregnancy, the latest in 29 weeks (average 23 weeks) .The surgical removal of pathological diagnosis of CCAM Ⅱ / ILS in 4 cases, of which 3 cases of mass more than one cyst ultrasound images, In the other 1 case, there was only lump and no hypersound pattern.All the lumps were located in the left lower lobe, at least one artery was seen in the lumps during operation, Only mass images without cystic ultrasound images
