目的研究翼状胬肉大小与角膜散光的关系,探讨胬肉手术的时机与必要性。方法:对52眼胬肉患者进行视力、屈光度及角膜曲率检查,其中对46眼行胬肉切除术者进行手术前后的上述3个指标的对比观察,并按胬肉大小分为两组(I组≤2.5 mm,II组>2.5 mm)进行对比观察。结果:52眼中均存在不同程度的散光,其中单纯远视散光占40.4%,混合性散光占28.8%,以顺规性散光为主占65.9%。胬肉切除术后散光度、垂直子午线的屈光力差均有明显降低P<0.001。视力平均提高2行,I、II组对比,II组改变显著,P<0.001。结论:胬肉手术能明显改善角膜的散光状况,从而提高视力,胬肉大小与角膜散光的发生关系密切,当其侵入角膜2.5~3.0 mm后有必要进行胬肉切除也是手术良机。
Objective To study the relationship between the size of pterygium and corneal astigmatism and to discuss the timing and necessity of pterygium surgery. Methods: Visual acuity, refraction and corneal curvature were examined in 52 patients with oculomotor pterygium. 46 patients underwent resection of pterygium for comparison of the above three indexes before and after surgery, and were divided into two groups according to the size of pterygium Group ≤ 2.5 mm, group II> 2.5 mm) were compared. Results: There were different degrees of astigmatism in 52 eyes, with hyperopic astigmatism accounting for 40.4%, mixed astigmatism accounting for 28.8%, and concomitant astigmatism accounting for 65.9%. Astigmatism after pterygium resection, the vertical meridian refractive power difference was significantly lower P <0.001. Visual acuity increased by 2 lines on average, I, II contrast, Group II changed significantly, P <0.001. CONCLUSION: Pterygium surgery can significantly improve corneal astigmatism, thereby improving visual acuity. The size of pterygium is closely related to the occurrence of corneal astigmatism. It is also an operation opportunity for the pterygium to be removed after corneal invagination is 2.5-3.0 mm.
