目的 :探讨以醛氢叶酸 (CF)与氟尿嘧啶 (5 FU)为主的化疗方案治疗晚期乳腺癌的增效作用。方法 :10 6例复发转移的乳腺癌患者 ,随机分成两组 ,A组 5 3例 ,应用CF +CAF方案 ,即醛氢叶酸 0 2 g/次 ,静脉滴注 ,第 1、2、3天 ;环磷酰胺0 5 g/m2 ,静脉注射 ,第 1、8天 ;阿霉素 35mg/m2 (或者表阿霉素 6 0mg/m2 ,或者吡喃阿霉素 35mg/m2 ) ,静脉注射 ,第 1天 ;氟尿嘧啶 0 5g/m2 ,静脉滴注 ,第 1、2、3天。 2 1天重复 ,连续 2~ 3周期为一疗程。B组 5 3例 ,单用CAF方案 ,药物剂量、用法、疗程同A组。结果 :A组近期有效率为 77 3% ,生活质量 (KPS)改善率为 75 4 % ,B组近期有效率为 5 6 6 % ,生活质量改善率为5 4 7% ,两组比较 ,两项指标均有显著性差异 (P <0 0 5 )。毒副反应 :A组的口腔炎、腹泻、白细胞数下降、静脉炎及皮肤色素沉着发生率高于B组 ,有显著性差异 (P <0 0 5或P <0 0 1) ,而脱发、心电图改变、血红蛋白量下降、血小板数下降及转氨酶增高等毒副反应 ,两组间比较无显著性差异 (P >0 0 5 )。结论 :对于晚期乳腺癌的治疗 ,CF能增加 5 FU的疗效 ,毒副反应亦有所增加。
Objective: To investigate the synergistic effect of chemotherapy regimen based on 5-FU and 5-FU in the treatment of advanced breast cancer. METHODS: A total of 106 breast cancer patients with recurrence and metastasis were randomly divided into two groups. 53 patients in group A were treated with CF +CAF regimen, that is, hypohydrocortisone 0.2 mg/dose, intravenous drip for 1, 2 and 3 days. ; Cyclophosphamide 0 5 g/m2, intravenous injection, the first 1 and 8 days; Adriamycin 35mg/m2 (or epirubicin 60mg/m2, or pirarubicin 35mg/m2), intravenous injection, Day 1; fluorouracil 0 5g/m2, intravenous drip, day 1, 2 and 3 days. 2 repeated 1 day, continuous 2 to 3 cycles for a course of treatment. In Group B, 53 patients were treated with CAF alone. The dose, usage, and course of treatment were the same as those in Group A. Results: The short-term efficacy rate in group A was 77 3%, the improvement rate in quality of life (KPS) was 754 percent, the short-term efficacy rate in group B was 56.6%, and the improvement rate in quality of life was 54.7%. There were significant differences in the indicators (P <0 05). Side effects: The incidence of stomatitis, diarrhea, leukocyte count, phlebitis, and skin pigmentation were higher in group A than in group B, with significant differences (P < 0.05 or P <0.01), and hair loss. Electrocardiogram changes, decreased hemoglobin, platelet counts, and increased transaminase toxicity were no significant differences between the two groups (P > 0.05). Conclusion : For the treatment of advanced breast cancer, CF can increase the efficacy of 5 FU, and the toxic and side effects also increase.
