目的研究脑出血后胃液pH值、胃粘膜细胞增殖与凋亡、胃粘膜血流量(GMBF)和前列腺素E2(PGE2)含量的变化,并探讨与胃粘膜损伤程度之间的关系。方法将88只Sprague-Dawley(SD)大鼠随机分为正常组、假手术组和模型组,后两者又分为6、12、24、48、96h组,每组各8只;在脑立体定向仪下采用自体动脉血注入右侧尾状核法制备大鼠脑出血模型,并在相应时间点处死大鼠;用放射免疫法检测胃粘膜PGE2含量,TUNEL法检测胃粘膜上皮细胞凋亡,免疫组化法检测上皮细胞增殖,并检测胃液pH值、GMBF及溃疡指数(UI)的变化。结果模型组胃粘膜pH值下降(P<0.05),GMBF明显减少(P<0.01),凋亡增多,增殖减少,PGE2含量下降(P<0.05),UI明显升高(P<0.01)。结论脑出血后胃粘膜发生明显损伤,其机制与胃粘膜pH值下降,细胞凋亡增多,GMBF下降,PGE2降低,细胞增殖减少密切相关。
Objective To study the changes of gastric mucosal pH value, gastric mucosal cell proliferation and apoptosis, gastric mucosal blood flow (GMBF) and prostaglandin E2 (PGE2) content after intracerebral hemorrhage, and to explore the relationship between gastric mucosal injury and gastric mucosal injury. Methods 88 Sprague-Dawley (SD) rats were randomly divided into normal group, sham operation group and model group. The latter two groups were divided into 6, 12, 24, 48, 96 h groups, The autologous arterial blood was injected into the right caudate nucleus to establish the model of intracerebral hemorrhage in rats under stereotactic apparatus. The rats were sacrificed at the corresponding time points. The content of PGE2 in gastric mucosa was detected by radioimmunoassay. The apoptosis of gastric epithelial cells was detected by TUNEL assay The proliferation of epithelial cells was detected by immunohistochemistry. The changes of gastric mucosal pH, GMBF and ulceration index (UI) were detected. Results The gastric mucosal pH decreased (P <0.05), the GMBF decreased (P <0.01), the apoptosis increased, the proliferation decreased, the content of PGE2 decreased (P <0.05), and the UI increased obviously. Conclusions After gastric hemorrhage, the gastric mucosa is obviously damaged. The mechanism is related to the decrease of gastric mucosal pH, the increase of apoptosis, the decrease of GMBF, the decrease of PGE2 and the decrease of cell proliferation.
