目的:研究支原体(MP)感染对儿童社区获得性肺炎(CAP)免疫功能的影响,以提高对儿童CAP及MP感染的认识。方法:回顾性分析2014年5月至2016年5月本院儿科CAP患儿161例,根据呼吸道病原体IgM抗体检测结果分为:MP阳性组45例,MP阴性组116例。比较两组患儿入院生命体征、体液免疫指标(IgG、IgA、IgM、补体C3、补体C4)及细胞免疫指标(CD3%、CD4%、CD8%、NK%、CD4/CD8)。结果:161例患儿MP感染总阳性率为28.0%,女孩MP阳性率显著高于男孩(38.8%vs 20.2%,P<0.05);对于体液免疫,MP阳性组IgM高于MP阴性组,有统计学差异(P<0.05),两组细胞免疫指标无统计学差异(P均>0.05)。结论:在儿童CAP中,女孩的MP阳性率高于男孩,而且MP感染主要影响患儿的体液免疫功能。
Objective: To study the influence of Mycoplasma (MP) infection on the immune function of children with community-acquired pneumonia (CAP) in order to raise awareness of CAP and MP infection in children. Methods: From May 2014 to May 2016, 161 children with CAP in our hospital were retrospectively analyzed. According to the results of respiratory tract pathogens IgM, there were 45 cases in MP positive group and 116 cases in MP negative group. The vital signs, humoral immunity indexes (IgG, IgA, IgM, complement C3, complement C4) and cellular immune indexes (CD3%, CD4%, CD8%, NK%, CD4 / CD8) of admission children were compared. Results: The positive rate of MP in 161 children was 28.0%. The positive rate of MP in girls was significantly higher than that in boys (38.8% vs 20.2%, P <0.05). For humoral immunity, IgM in MP positive group was higher than that in MP negative group (P <0.05). There was no significant difference in cellular immunity between the two groups (P> 0.05). Conclusion: In children with CAP, the positive rate of MP in girls is higher than that in boys, and MP infection mainly affects the humoral immune function in children.
