目的 探究急性缺血性脑卒中(acute ischemic stroke,AIS)患者白细胞计数水平与早期神经功能恶化(early neuro-logical deterioration,END)的关系.方法 回顾深圳市某三甲综合医院神经内科2014年1月 ～2018年12月收治的AIS患者临床资料,依据NIHSS评分,将患者分为非END组294例,END组64例,并分析白细胞计数水平与END的关系.结果 采用四分位数法将基线白细胞计数分为Q1(<6.18×109/L)、Q2(6.18×109/L～7.47×109/L)、Q3(7.48×109～9.41×109/L)和Q4(≥9.42×109/L)4组,调整入院体温、心房颤动史、颈动脉中重度狭窄以及血尿酸等混杂因素后,多因素非条件Logistic回归分析显示,Q3、Q4组AIS患者发生END的风险分别是Q1组的5.226倍(OR=5.226,95％CI:1.782～15.325,P=0.003)和5.489倍(OR=5.489,95％CI:1.918～15.709,P=0.002).结论 白细胞计数水平是AIS患者END的独立危险因素,且白细胞计数水平的增高能够增加END的发生风险,为临床医护人员加强对白细胞计数的监测及相关炎性反应的控制提供参考依据.
