目的探讨无创正压机械通气治疗支气管哮喘急性发作合并呼吸衰竭的临床效果。方法 70例支气管哮喘急性发作合并呼吸衰竭患者,随机分为观察组和对照组,每组35例。两组均给予常规治疗,对照组给予面罩或鼻导管吸氧,观察组实施无创正压机械通气。观察两组患者治疗前和治疗48 h后心率、呼吸频率、动脉血气指标改变情况,观察两组患者实施有创机械通气情况。结果观察组治疗48 h后的心率、呼吸频率均低于对照组(P<0.05);观察组治疗48 h后氧分压、二氧化碳分压、p H值与对照组比较差异有统计学意义(P<0.05);观察组实施有创机械通气发生率(5.71%)低于对照组(25.71%),差异有统计学意义(P<0.05)。结论无创正压机械通气治疗支气管哮喘急性发作合并呼吸衰竭临床效果显著,值得借鉴。
Objective To investigate the clinical effect of noninvasive positive pressure mechanical ventilation in treating acute exacerbation of bronchial asthma with respiratory failure. Methods Seventy patients with acute exacerbation of bronchial asthma with respiratory failure were randomly divided into observation group and control group with 35 cases in each group. Both groups were given conventional treatment, the control group were given mask or nasal oxygen catheter, the observation group to implement noninvasive positive pressure mechanical ventilation. The changes of heart rate, respiration rate and arterial blood gas index were observed before treatment and after 48 hours of treatment. The invasive mechanical ventilation was observed in both groups. Results The heart rate and respiratory rate in observation group were lower than those in control group at 48 h (P <0.05). The oxygen partial pressure, carbon dioxide partial pressure and p H value in observation group after 48 h treatment were significantly different from those in control group P <0.05). The incidence of invasive mechanical ventilation in the observation group (5.71%) was lower than that in the control group (25.71%), the difference was statistically significant (P <0.05). Conclusion Noninvasive positive pressure mechanical ventilation in the treatment of acute exacerbation of bronchial asthma with respiratory failure has a significant clinical effect and is worth learning from.
