背景在埃及,家庭医学(FM)专业对医学教育来说是个新鲜事物,家庭医学职业选择情况还不明朗。本研究探讨了实习医生对家庭医学的认知、期待、信息层次和职业选择情况。方法本研究为横断面描述性研究,在实习医生参与初级医疗工作的入职培训期间进行,采用匿名自填调查问卷采集数据。结果超过50%的调查对象认为家庭医学有很重要的社会功能。对于实习医生如何看待家庭医学,最重要的影响因素是学习过程中的自我体验(52.6%)和对家庭医生的看法(43.8%)。超过40%的调查对象认为卫生服务中心的培训课程是有益处的,29.5%认为家庭医学实习培训量在医学院总实习培训量中至少应该占25%~50%。将近一半(48.2%)的调查对象建议家庭医学实习培训应在大四期间进行。在入职教育之前,15.8%的调查对象表示会选择家庭医生作为职业,在入职培训后,这一比例增加到50.4%。农村居住和公共卫生科目低分是选择家庭医生作为职业的独立预测因素(OR值分别为1.9、1.7)。结论家庭医学正在迅速发展为一项专业。经常开展针对实习医生的培训项目会鼓励实习医生选择家庭医学作为职业。
Background In Egypt, the specialty of home medicine (FM) is new to medical education, and the choice of career in family medicine is unclear. This study explored the interns’ perceptions, expectations, levels of information and career choices for family medicine. Methods This study, a cross-sectional descriptive study, was conducted during the on-the-job training for trainees engaged in primary care and collected data using anonymous self-completed questionnaires. Results More than 50% of respondents think that family medicine has important social function. The most important influencing factors for intern practitioners on family medicine are self-experience during learning (52.6%) and family doctor (43.8%). More than 40% of the respondents considered the health service center training courses are beneficial, 29.5% think that the training of home medical practice should be at least 25% to 50% of the total intern training in medical school. Nearly half (48.2%) of the respondents suggested that family medicine training should be conducted during the senior year. Before induction education, 15.8% of the respondents indicated that they would choose a GP as their occupation and this proportion increased to 50.4% after induction training. A low score for rural living and public health subjects was an independent predictor of the choice of family doctor as a career (OR = 1.9, 1.7, respectively). Conclusion Family medicine is rapidly evolving into a specialty. Regular training programs for interns encourage interns to choose home medicine as a career.
