目的:对化学发光检测的梅毒抗体S/CO低值样本采用不同方法进行分析及评价。方法:于2017年7至2019年9月对桂林医学院附属医院检验科采用化学发光微粒子免疫分析法(chemiluminesent micropaticle immunoassay, CMIA)检测的1050例梅毒抗体弱阳性血清标本分别用梅毒螺旋体明胶凝集试验(Treponema pallidum particle agglutination test,TPPA)及免疫胶体金法进行复检,并进行免疫印迹检测。结果:对于S/CO值在1.01~9.21之间样本,随着化学发光S/CO值的增高,其与TPPA法与胶体金法的阳性符合率逐渐提高。对于S/CO值在1.01~2.00之间标本,CMIA法与TPPA法及胶体金法阳性符合率分别为43.55%和37.10%,经梅毒抗体免疫印迹法(syphilis antibody western blot,TP-WB)验证后,CMIA低值样本的阳性预测值为32.38%。CMIA法S/CO弱阳性人群中,≥70岁老年人群与0.05)。n 结论:CMIA法筛查梅毒抗体S/CO低值的样本,阳性预测值较低,复检有助于提高检测结果的准确性。“,”Objective:Using various methods to remenasure samples with a low S/CO ratio syphilis antibody detected by chemiluminesent micropaticle immunoassay(CMIA)in order to explore and evaluate clinical detection of syphilis antibody.Methods:From July 2017 to September 2019, 1050 cases of syphilis antibody weak positive serum samples detected by CMIA were retested by Treponema pallidum particle agglutination test (TPPA) and immunogold method respectively, and immunoblotting was performed.Results:For the samples with S/CO value between 1.01~9.21, with the increase of chemiluminescence S/CO value, The positive coincidence rate with TPPA and colloidal gold method increased gradually. For the samples with S/CO value between 1.01~2.00, the positive coincidence rate of CMIA with TPPA and immunogold was 43.55% and 37.10%, respectively. After confirmed by tp-wb, the positive predictive value of CMIA low value samples was 32.38%. There was no difference in the positive rate of TPPA reexamination between the aged≥70 and the aged0.05).n Conclusion:Remeasurement of samples, which have low S/CO ratio syphilis antibody by CMIA, could improve the accuracy of the test results.
