弥漫大B细胞淋巴瘤(diffuse large B cell lymphoma,DLBCL)是一种常见的非霍奇金淋巴瘤(non-Hodgkin lymphoma,NHL).利妥昔单抗的出现极大的改善了DLBCL患者的预后.对于DLBCL预后的评估,目前临床上应用的是国际预后指数(International Prognostic Index,IPI),但IPI仅是部分临床指标的结合,单纯依靠IPI评估预后已经不能满足临床需要.总结了4例经我院诊治的广泛浸润的DLBCL患者,所有患者为IPI评分中高危或高危,同时还具有肿瘤负荷大、累及范围广、血清白蛋白明显减低、Ki-67较高、多为非生发中心来源等特点,都提示预后不佳,但经过利妥昔单抗联合CHOP方案治疗后均获得了良好的疗效.因此,联合血液科、病理科、检验科对本组患者诊治与预后进行多学科讨论,进一步探讨弥漫大B细胞淋巴瘤的预后评价指标和治疗手段的选择,推动多学科间的合作.
