Fatigue in Woman with Iron Deficiency without Anemia: Utopia or Reality?

  Iron is one of the cornerstones of cellular function being key in hemoglobin and DNA synthesis and influencing enzymatic activity and especially mitochondrial function.The supposed association between iron deficiency and fatigue is long standing and has so far been difficult to prove.Whereas fatigue in the context with iron deficiency "anemia" is self-evident for every doctor and notably pathophysiologically easy to comprehend, fatigue associated solely with low iron stores in "non"-anemic patients is unfamiliar and more difficult to understand.In a double-blinded, placebo-controlled study, we randomized 90 premenopausal women presenting with fatigue, serum ferritin-3d 50 ng/mL, and normal hemoglobin (≥3d 120 g/L) to receive either 800 mg of intravenous iron (Ⅲ)-hydroxide sucrose or intravenous placebo.In patients with baseline serum ferritin-3d 15 ng/mL, fatigue decreased by 1.8 in the iron group compared with 0.4 in the placebo group (P =.005), and 82% of iron-treated compared with 47% of placebo-treated patients reported improved fatigue (P =.03).Intravenous administration of iron improved fatigue in iron-deficient, but not anemic women with a good safety and tolerability profile.The ameliorating effect of iron therapy on fatigue in women with low iron stores but normal hemoglobin may change the paradigm that fatigue in iron deficient women is solely a function of anemia.Given the high number of women who are iron deficient and fatigue, the new finding could find broad application.
