Purpose: To prospectively assess the value of postmortem fetal MRI following stillbirth or termination of pregnancy (TOP) for medical reasons.Study Data: Between December 2008 and August 2011, postmortem fetal MRI was performed in 150 pregnancies following the diagnosis of stillbirth (n=32) or TOP for medical reasons (n=118).Informed consent was obtained for all MRI exams.There were 138 singleton, 11 twin and 1 triplet pregnancies.Gestational age (GA) ranged between 12 and 39 weeks (median=22).TOP was performed by using prostaglandin induction for GA < 22 weeks and fetocide + prostaglandin induction for GA > 22 weeks.All fetuses (n=161) were scanned on a 3 Tesla MRI scanner with high resolution (isotropic voxel size ranging between 0.3 and 0.5 mm).The median time delay between delivery and postmortem MRI was 8 hours (range 0.5-24 hours).Postmortem radiographies were taken for all fetuses.Prenatal ultrasound (US) was available for all cases, intrauterine fetal MRI data obtained in 42 cases (=28%).Conventional autopsy following virtopsy was performed in 117 cases (=78%).Most frequent observed fetal anomalies on the postmortem MRI scans were central nervous system (48%), musculoskeletal system (37%), urogenital system (29%), respiratory system (29%), digestive (23%), cardiovascular (20%) and pharyngeal (branchial) apparatus system (18%).Discussion: The correlation between prenatal imaging data (US and MRI), postmortem MRI and conventional autopsy will be reviewed, with emphasis on central nervous system anomalies.The strengths and weaknesses of conventional versus virtual autopsy examination techniques will be discussed.
