Background: Studies have shown a wide range of prevalence in the psychiatric disorders found among the elderly; prevalence has ranged from 42.1% to 96.5%.The aim of the present study was to estimate the prevalence of psychiatric disorders at death among elderly suicides in Australia, when compared to controls and middle-aged suicides.Methods: The Psychological Autopsy method was utilised to investigate suicides over the age of 35 from two Australian sites by interviewing next-of-kin and healthcare professionals in 2006-2008.A case-control study design was applied using sudden deaths as controls.Initial information was gathered from the coroners office.Potential informants were approached and interviews were conducted by clinical interviewers in a semi-structured format using validated scales.Results: In total, 261 suicides (73 aged 60+) and 182 sudden deaths (79 aged 60+) were involved.Elderly suicides had a lower prevalence of psychiatric diagnosis (62%) when compared to the middle-aged suicides (80%).Mood disorders were most frequently found in both suicide groups.Substance-related and psychotic disorders were significantly more prevalent in the middle-aged suicides.In both age groups, suicides were significantly more likely to have a psychiatric diagnosis at death, compared to the controls.Mood disorders were significantly more frequently found in the suicide groups.Suicides were more likely to have anxiety disorders and substance-related disorders, compared to the controls; difference did not reach significance for the elderly group.Suicides in the elderly group had a lower proportion of dementia, compared to the controls.Conclusion: The prevalence of psychiatric diagnoses in elderly suicides was lower in our study, when compared to the earlier studies, but still higher compared to the more recent studies.This indicates the need to further study mentally-well elderly suicides in Australia.
