Bisphosphonates are now commonly prescribed for the treatment of osteoporosis and bone cancers.Bisphosphonateinduced osteronecrosis of the jaws (BIONJ) refers to a condition characterized by persisted necrosis and exposure of the bone in the jaws of patients who had been experienced or currently taking a bisphosphonate without head and neck radiation therapy.Risedronate (Actonel(r)), an oral form bisphosphonate, has become more prevalent but discover few reports ofBIONJ.A seventy two years old woman came to our department with an unhealed socket after removal of# 46 for 3 months.She had been treated osteoporosis with weekly Risedronate for 2 years.(Prednisolone and metrotrexate have started since last year) Previously medical history was rheumatoid arthritis, osteoporosis and a current history ofprednisolone and metrotrexate use.The patient was performed four times local debridement and sequestrectomy with primary closure under local anesthesia, antibiotic was prescribed and also cessation ofrisedronate, metrotrexate, prednisolone.The wound was completely healed 6 months after first operation.This patient had multiple problems and treated by various medications, which increased risk of BRONJ.Multidisciplinary approach was important to reach the success of treatment while medications adjustment and surgical treatment would have to be synchronized.
