Morbidity and mortality rates following ST-elevation myocardial infarction have fallen substantially in the past few decades with improvements in medical care, particularly the widespread use of reperfusion techniques (fibrinolysis and primary percutaneous coronary intervention [PPCI]) and the adjunctive use of multiple medical therapies.On the basis of the data from several randomised studies, guidelines have supported the superiority of PPCI over fibrinolysis, if the door-to-balloon is completed in a timely fashion.However, in a significant proportion of patients undergoing PPCI, owing to a high thrombus burden, myocardial perfusion is not fully achieved in the epicardial vessel.
