　　Study Design:The occupancy rate of ossification of the posterior longitudinal ligament (OPLL) on CT scan,sagittal diameter of spinal cord on MRI and JOA score before and after anterior extirpated decompression in a consecutive patients were analyzed and compared in a retrospective study.Objective:To report the effect of anterior extirpated decompression for treating OPLL with average occupancy ratio of the spinal cord exceeding 65%.Summary of Background Data:Cervical myelopathy caused by OPLL can be treated by anterior approach or posterior approach.The posterior approach is often demanding for patients with continuous type and or with occupancy rate exceeding 50%.But as it is not an extirpate method that do not remove the ossified ligament directly,the anterior decompression is more theoretically rational for radical removal of the ossified ligament despite its high demanding of technique.Few studies had documented the results of anterior approach for treating massive OPLL that with occupancy ration exceeding 50%.
