超声造影对乳腺良恶肿瘤的鉴别诊断价值的研究

  目的 进一步研究超声造影在乳腺病灶鉴别诊断中的应用价值.方法 以病理结果为金标准对照本研究对术前261例中的307个乳腺病灶在手术前实施超声造影检查,并应用Sonoliver软件进行时间-强度曲线分析,在观察中发现对较小的低回声实质病灶,实施造影检查有助于提高对病灶良恶性的鉴别诊断.结果 本文结合病理结果回顾性总结造影后良恶性肿瘤增强模式表现为:186个良性病灶在超声造影早期表现为均匀增强者134个、不均匀增强52个.其中21个为高增强、36个为中增强、74个为弱增强、55个为无增强,186个均无周边放射状增强;121个恶性病灶在超声造影早期表现为快速均匀增强47个、非均匀增强74个,其中78个为高增强、21个为中增强、18个为弱增强、4个为无增强;97个表现为周边放射状增强.乳腺恶性肿瘤的不均匀增强造影模式考虑与病灶合并坏死液化有关.造影结果与病理结果对照,186个良性病灶166诊断正确,20个误诊为恶性病灶;121个恶性病灶103个诊断正确,有18个误诊为良性.对恶性肿瘤敏感性为85.1%,特异性为89.2%,准确性为87.6%.结论 本文提示恶性肿瘤的超声造影表现为病灶早期快速非均匀高增强,周边放射状增强.而良性肿瘤则表现为病灶早期缓慢均匀弱增强和无增强,均无周边放射状增强;乳腺恶性病灶的超声造影表现为病灶早期快速非均匀高增强,周边放射状增强.良性病灶则表现为病灶早期缓慢均弱增强和无增强,均无周边放射状增强;乳腺病灶造影后长/宽比显著减小考虑恶性肿瘤,恶性病灶感兴趣区的峰值强度比值与良性病灶相比有差异.超声造影可以动态观察乳腺病灶内微血管血流灌注状况,应用Sonoliver软件可进一步对病灶进行量化分析、有助于提高乳腺良恶性病灶的鉴别诊断水平.
