Objective: to elucidate risk factors for macrovascular disease in elderly people with diabetes mellitus.Methods: A randomized, controlled, multi-centre, prospective intervention trial was performed.A total of 1173 elderly patients (aged 65 years) with type 2 diabetes mellitus from 39 Japanese institutions were participated in the study.A landmark survival analysis was performed in which follow-up was set to start at 1-year after the initial time of entry.Results: 1) There was no significant difference in fatal and non-fatal events between intensive and conventional treatment groups.2) During 6 years of follow-up, 69 non-fatal and fatal strokes, 55 cardiovascular events, 35 diabetes-related deaths, and 155 total diabetes-related events occurred.3)A high LDL-cholesterol level predicted incident cardiovascular events, whereas high non-HDL-cholesterol level was one of independent predictors for stroke, and diabetes-related mortality.4) Although high HbA1c and systolic blood pressures were associated with incident stroke, a J-curve incidence for stroke was observed according to HbA1c and systolic BP distributions.5) Low physical activity assessed with Baecke questionnaire is also one of predictors for stroke.Conclusions: Comprehensive management of glucose, blood pressures, and lipids as well as exercise promotion may be of great importance even for the prevention for macrovacular disease in elderly people with diabetes mellitus.
