Glycemic control is fundamental in the management of diabetes but is also essential for patients suffering from stress hyperglycemia as seen in surgical patients, critically ill patients, wasting cancer patients or patients on certain medication like steroids.Traditionally most attention has been given to fasting glucose levels in order to improve glycemic control.However in recent years management of postprandial glucose levels has been recognized as an important complementary strategy to achieve optimal glycemic control.We have studied the significance of diabetes-specific tube feeding for the management of patients with disturbed glucose response and in need of nutritional support.We showed that compared to normal feeding the use of low glycemic diabetes specific feeding results in improved 24h glucose profiles after bolus feeding as well as after continuous feeding;incrementalArea Under the Curve (iAUC) decreased with 44% (P<0.05), and 38% (P<0.01) respectively.Furthermore we have generated data in type Ⅱ diabetes patients that show that, also after longer term use of a diabetes-specific tube feed, the postprandial glucose responses (iAUC) remained to be lower compared with an isocaloric, fiber-containing, standard tube feed.With this improved glycemic control, HbA 1C significantly decreased over time and significantly reduces HbA1C in the diabetes-specific group and not in the standard feed group (Treatment * time: P=0.034).We conclude that low glycemic diabetes-specific tube feeding is the preferred choice for nutritional support in patients with elevated glucose levels.
