目的利用非接触式自动角膜内皮计对较大样本中老年人角膜内皮细胞和角膜中央厚度进行分析,获得其结构参数的生理正常值范围。方法对670例中老年白内障患者术前角膜内皮检查资料进行回顾性分析。206例采用Topcon内皮计,464例采用Humphrey内皮计。相关结构参数包括内皮细胞密度,内皮细胞平均面积,最大和最小面积,面积标准差和变异系数,六边形细胞所占比例,以及中央角膜厚度。结果统计资料平均年龄(69±9)岁(41~94岁)。Topcon内皮计和Humphrey内皮计单纯白内障患者平均角膜内皮细胞密度分别为(2683±417)个/mm2和(2696±404)个/mm2,均有随年龄增长而下降的趋势,60岁上下出现较显著性差异。内皮细胞平均面积分别为(381±65)μm2和(381±75)μm2。Topcon内皮计测得平均中央角膜厚度为(0.541±0.037)mm。Humphrey内皮计测得六边形细胞所占比例为(53±10)%。白内障合并青光眼患者65例,其平均细胞密度显著低于单纯白内障患者(p<0.01),平均细胞面积显著增大(p<0.01)。结论中老年人眼角膜内皮细胞数量和大小仍随年龄变化而改变,60岁上下出现较显著性差异。了解不同年龄段人群的角膜内皮参数正常值有助于为各类眼病治疗或内眼手术提供安全保障。Topcon内皮计和Humphrey内皮计的测量结果具有可比性。
Objective To analyze the central corneal endothelial cells and the central corneal thickness in large samples by non-contact automatic keratometer, and to obtain the physiological normal range of the structural parameters. Methods A retrospective analysis of 670 cases of cataract patients with preoperative corneal endothelial examination data were analyzed. 206 cases with Topcon endothelium, 464 cases with Humphrey endothelium. Related structural parameters include endothelial cell density, average endothelial cell area, maximum and minimum area, area standard deviation and coefficient of variation, proportion of hexagonal cells, and central corneal thickness. Results The average age of statistical data (69 ± 9) years (41 to 94 years). The mean corneal endothelial cell densities of Topcon endothelium and Humphrey endothelium were (2683 ± 417) / mm2 and (2696 ± 404) / mm2, respectively, with the trend of declining with age. Significant difference. The average area of endothelial cells were (381 ± 65) μm2 and (381 ± 75) μm2, respectively. The average central corneal thickness measured by Topcon endothelium was (0.541 ± 0.037) mm. Humphrey endothelium measured hexagonal cells accounted for (53 ± 10)%. 65 cases of cataract combined with glaucoma, the average cell density was significantly lower than that of cataract patients (p <0.01), the average cell area was significantly increased (p <0.01). Conclusions The number and size of corneal endothelial cells in middle-aged and elderly people still change with age. There is a significant difference between 60 and 60 years old. Understanding the normal corneal endothelial parameters of people of different ages helps to provide safety and security for the treatment of various types of eye diseases or for endoscopic surgery. Topcon endothelium and Humphrey endothelium measurement results are comparable.
