目的 观察人体静注利多卡因对脑氧耗和脑血管功能的影响。方法 20 例颅脑手术病人,麻醉前经颈内静脉和桡动脉穿刺置入导管。在静注2% 利多卡因1.5m g/kg 前后2 分钟分别从导管中取动、静脉血标本检测血气。并用HNCPA-2脑血管功能检测仪记录双侧颈内动脉和椎动脉的分钟血流量、血流入时间和血管转折高比率。用公式计算脑氧耗量。结果 注药后脑氧耗量降低17.6% ,入颅动脉血管弹性降低9.8% ,血液流入阻力增加17.6% ,脑血流量减少13% 。结论 单次静注利多卡因后出现脑血管收缩,入颅血流量和脑氧耗量降低,有利于降低颅内压和保护颅脑手术中缺血脑细胞的功能。
Objective To observe the effect of intravenous injection of lidocaine on cerebral oxygen consumption and cerebrovascular function. Methods Twenty patients with craniocerebral surgery were catheterized by internal jugular vein and radial artery before anesthesia. In the intravenous injection of 2% lidocaine 1.5m g / kg before and after 2 minutes were taken from the catheter, venous blood samples were tested for blood gas. HNCPA-2 cerebrovascular function detector was used to record the minute blood flow, the time of inflow of blood and the high rate of vascular turnover in bilateral carotid artery and vertebral artery. Use formula to calculate brain oxygen consumption. Results After injection, the cerebral oxygen consumption decreased by 17.6%, the elasticity of the cranial artery reduced by 9.8%, the resistance to blood flow increased by 17.6% and the cerebral blood flow decreased by 13%. Conclusion After a single injection of lidocaine, cerebral vasoconstriction, decreased blood flow to the skull and decreased brain oxygen consumption are helpful to reduce intracranial pressure and protect ischemic brain cells during craniocerebral operation.
