中西医结合治疗严重外伤性前房积血太原市中心医院（０３０００９）冯立言太原市人民医院张香梅笔者采用中西医结合治疗严重外伤性前房积血３例，取得满意效果，现报道如下。资料与方法３例严重性前房积血均为男性；年龄１８～２３岁；右眼１例，左眼２例；致伤原因：２例为拳击伤，一例为砖块砸伤；积血量１例能见上端有一线状水平面，２例充满整个前房；均于受伤当天接受治疗。治疗方法：（１）西药常用安络血、维生素Ｋ、维生素Ｃ等；（２）中药治疗：①早期：对新鲜出血以凉血止血为主。方剂：生地、大蓟、小蓟、白茅根、茜草、丹皮、阿胶、地榆炭、仙鹤草、侧柏叶、黄芩加减；②中晚期：对较陈旧的出血、可用活血化淤和滋补血之类调理药，常用中药者。当归、川芎、赤芍、丹参、桃仁、白芷、荆芥、泽兰。③积血基本吸收后，根据情况加服六味地黄丸、杞菊地黄丸等巩固疗效。结果：３例经５～１４天治疗前房积血均获吸收，视力均恢复至１．０以上。讨论机械性眼外伤前房积血，祖国医学认为是“血灌瞳神”，其病变主要在水轮和风轮部位，因外伤撞击，内损血络，血溢络外，灌入瞳神。我们认为本病早期即出血期，是不经血运，血热妄行，用凉血止血药为主。在出血稳定好转期，可用活血化淤药。后期根据病
