例1 男,8岁。反复咳喘8年加重伴心慌气急1年。患儿生后次日即患肺炎,平时极易咳喘,发热,曾因肺炎11次住院。近1年来咳喘频繁,活动后心慌,气急。体检:端坐呼吸,颈静脉怒张,鸡胸,桶状胸。双肺语颤减弱,过清音,满布哮鸣音,肺底细湿啰音?M幌驴杉牟?P_2亢进。肝肋下3cm、肝颈征(+),肢端发绀,杵状指(+)。实验室检查:ESR14mm/小时;痰查含铁血黄素颗粒3次(-);血清蛋白电泳:Alb46%、α_11.2%、α_26.5%、β9.7%、γ36.6%;抗核抗体2次(+);类风湿因子2次(+);免疫球蛋白测定:IgG19.5g/L、IgA3.25g/L、IgM1.52g/L;
