患者男性,73岁,心动过缓8年,晕厥11次。入院。患者于8年前发现脉搏缓慢(30次/分),心电图检查为Ⅱ°AVB,BP29/131kPa无自觉不适。使用异丙肾素时心率可增加最快可达60次/分。常无诱因突然晕倒,意识丧失,1—2分钟后自行苏醒。查体一般情况好,BP26/11kPa。肺气肿征阳性,双肺底少许湿罗音。心界向左增大,心率36次/分,律齐,心尖区可闻大炮音,心电图为Ⅲ°
