目的:通过检测慢性乙型肝炎患者干扰素(IFN)治疗前后HBV DNA前C区1896位变异,探讨其与IFN治疗之间的关系。方法:采用PCR-RFLP法检测慢性乙型肝炎患者前C区1896位变异,观察IFN治疗前后变异株的变化及IFN疗效。结果:200例慢性乙型肝炎患者IFN治疗前变异株检出率为19%,IFN治疗后未检出新出现的变异株;变异株、野毒株感染的慢性乙型肝炎患者IFN治疗有效率分别为60%与50%,均显著高于对照组(P=0.007,P<0.001),变异株、野毒株感染的慢性乙型肝炎患者IFN治疗有效率比较无差异(P>0.05)。结论:IFN治疗无诱导前C区1896位变异的作用;干扰素治疗变异株病毒感染的慢性乙型肝炎有效,其近期疗效与野毒株感染的慢性乙型肝炎基本相同。
OBJECTIVE: To investigate the relationship between pre-and post-HBV DNA pre-and post-HBV genotype 1896 mutation in patients with chronic hepatitis B and IFN treatment. Methods: PCR-RFLP was used to detect the 1896-bp mutation in pre-C region of patients with chronic hepatitis B. The changes of IFN-γ and the variation of IFN before and after treatment were observed. Results: The detection rate of IFN before treatment in 200 patients with chronic hepatitis B was 19%, and no new mutation was found after IFN treatment. The effective rate of IFN treatment in patients with chronic hepatitis B infection (P = 0.007, P <0.001). There was no significant difference in the effective rate of IFN treatment between chronic hepatitis B patients infected by mutants and wild strains (P> 0.05). CONCLUSION: IFN treatment did not induce the mutation of 1896 before C region. Interferon was effective in treating chronic hepatitis B virus infection. The recent curative effect was basically the same as that of chronic hepatitis B infected with wild strain.
