病史摘要马某,男,1 2/12岁,呕吐咖啡色物3～4天伴便血。患儿发病前半月曾呕吐咖啡色物及黑便2次。体检:体温、呼吸、脉搏和血压均正常,体重8.5公斤。神志萎靡,营养状况欠佳,皮肤粘膜苍白,无黄染,双肺无异常,心脏听诊(-),腹软,肝、脾肋下未及。白细胞7.0×10~9/L,血红蛋白10.29g％,血小板37.6×10~9/L,红细胞4.93×10~(12)/L。肝、肾功能均正常。大便隐血(+++)。胃肠钡餐示幽门梗阻。剖腹探查,幽门前胃窦小弯侧慢性溃疡伴疤痕性幽门梗阻。病理诊断(病理号920363):胃小弯多发性溃疡。讨论儿童消化性溃疡发生于2周岁以下幼儿罕见。一般来说,年龄愈小,愈不典型。故对疑似病例
