目的:观察经腹L型切口行巨大肾肿瘤切除术的疗效。方法:针对15例肿瘤体长直径在5-27cm肾肿瘤患者行经腹L型切口巨大肾肿瘤切除术。结果:顺利完成15例手术,手术时间在90-120min,平均为100min,术中输血0-400ml,平均200ml,术后致肛门排气时间为48-96h,平均72h,切口拆线时间平均为9天,1例瘤体与脾静脉粘连撕裂,术中出血,行缝扎止血。术后均未有并发症发生。结论:经腹L型切口行巨大肾肿瘤切除治疗术野显露满意,便于处理肾蒂以及腹腔里粘连浸润的部位,易于淋巴结清扫,操作可靠安全。
Objective: To observe the curative effect of giant renal tumor resection by transabdominal L-type incision. Methods: Fifteen patients with long tumors of 5-27 cm in diameter were treated with transabdominal L-shaped incision of giant renal tumor. Results: 15 cases were successfully completed. The operation time was 90-120min with an average of 100min. The intraoperative blood transfusion was 0-400ml with an average of 200ml. The postoperative anal exhaust time was 48-96h with an average of 72h. 9 days, 1 case of tumor and splenic vein adhesion tear, intraoperative bleeding, suture to stop bleeding. No postoperative complications occurred. Conclusion: The transabdominal L-type incision is performed satisfactorily in the treatment of giant renal tumor. It is easy to treat the renal pedicle and the site of adhesion infiltration in the peritoneal cavity. It is easy to be dissected and the operation is reliable and safe.
