目的探讨高敏肌钙蛋白I(h s-c T n I)在急性冠脉综合征(A C S)患者中的临床诊断以及预测患者冠状动脉病变的严重程度。方法选择初步诊断为A C S且接受冠脉造影检查的7 2例患者作为观察组,于冠脉造影前采血测定h s-c T n I;选择排除心血管疾病的3 5例患者作为对照组,并测定h s-c T n I。根据冠脉造影结果,将观察组分为血管狭窄程度<7 0%组和血管狭窄程度≥7 0%组,同时根据狭窄累及血管范围分为单支病变组和多支病变组,分析其对应的h s-c T n I水平。结果对照组和观察组患者的血清h s-c T n I经秩和检验分析,差异有统计学意义(P<0.0 1);观察组中,单支病变组与多支病变组患者血清h s-c T n I水平经秩和检验分析,差异无统计学意义(P>0.5);血管狭窄程度<7 0%组与血管狭窄程度≥7 0%组患者血清h s-c T n I水平经秩和检验分析,差异有统计学意义(P<0.0 1)。结论检测h s-c T n I的水平可有效预测患者冠状动脉病变严重程度以及冠状动脉病变的范围,并可作为A C S早期危险分层的一个可靠指标,对临床诊疗有一定的参考价值。
Objective To investigate the clinical significance of hs-cTnI in patients with acute coronary syndrome (ACS) and to predict the severity of coronary lesions in patients. Methods Seven hundred and two patients initially diagnosed as ACS and undergoing coronary angiography were selected as the observation group. Blood samples were collected before coronary angiography. H sc T n I; 35 patients with cardiovascular diseases were excluded as control group, h sc T n I. According to the results of coronary angiography, the observation group was divided into the group of stenosis of less than 70% and the group of stenosis of more than 70% of the stenosis. According to the range of stenosis and vessel involvement, the observation group was divided into single-vessel disease group and multi-vessel disease group, H sc T n I levels. Results In the control group and observation group, the serum h sc T n I levels were analyzed by rank sum test, the difference was statistically significant (P <0.0 1). In the observation group, serum h sc T The rank of n I in patients with vascular stenosis less than 70% and the degree of stenosis more than 70% were analyzed by rank sum test, the difference was not statistically significant (P> 0.5) , The difference was statistically significant (P <0.0 1). Conclusion The detection of hs-cTn I level can effectively predict the severity of coronary artery disease and the extent of coronary artery disease, and can be used as a reliable indicator of the early risk stratification of A C S, which is of certain reference value for clinical diagnosis and treatment.
