目的:研究升降散对亚甲型流感病毒鼠肺适应株FM1感染小鼠免疫功能的影响。方法:建立亚甲型流感病毒鼠肺适应株FM1感染小鼠肺炎模型,用升降散全方、升降散拆方、升降散与麻黄杏仁甘草石膏汤合方治疗后,第5天用ELISA方法检测小鼠支气管肺泡、大小肠灌洗液中sIgA、血清中炎性细胞因子IL-1β、IL-6、TNF-α、IL-10、IL-lra、sTNFR的含量;流式细胞仪法测定外周全血中T细胞亚群CD4~+、CD8~+的百分比和CD4~+/CD8~+值;计算胸腺指数和脾指数。结果:模型组小鼠较正常组小鼠支气管肺泡、大小肠灌洗液中sIgA含量显著降低,血清IL-1β、IL-6、TNF-α、IL-10、IL-lra、sTNFR的含量显著升高,全血CD8~+的百分比增高,CD4~+百分比和CD4~+/CD8~+值下降,胸腺指数明显降低,但对脾指数无明显影响。升降散(4.55g/kg)能显著提升支气管肺泡、大小肠灌洗液中sIgA含量,降低血清IL-1βIL-6,TNF-α含量,升高IL-10、IL-lrα、sTNFR的含量、全血CD4~+百分比、CD4~+/CD8~+值及胸腺指数,升降散组对这些指标的改善作用优于拆方组而逊于合方组。结论:升降散增加黏膜分泌IgA,调控炎性因子和抗炎因子的平衡及提高T细胞亚群的比值,是其改善肺部炎症的机制之一,也是升降散治疗病毒性肺炎效用机制的初步阐明。
