目的:探讨采用HC2方法检测人乳头瘤病毒DNA(HPV-DNA)载量与妇女宫颈病变严重程度的相关性。方法:选取本院妇科门诊收集的慢性宫颈炎患者70例、宫颈上皮轻度不典型增生(CIN 1)40例、中度不典型增生(CIN 2)40例、重度不典型增生(CIN 3)40例、宫颈癌40例,分别对患者进行HC2检测,分析不同类型宫颈病变与HPV-DNA载量的关系。结果:宫颈炎病变组的HPV-DNA阳性率为51.43%显著低于CIN 1(80%)、CIN 2(95%)、CIN 3(95%)、宫颈癌组患者(82%)(P<0.05),CIN 1组患者HPV-DNA阳性率显著低于CIN 2、CIN 3组患者(P<0.05)。宫颈炎组患者的HPVDNA载量分级与宫颈炎无显著的相关关系(P>0.05),CIN 1、CIN 2、CIN 3、宫颈癌4个组患者的病变程度均与HPV-DNA载量分级具有显著的相关性(P<0.05);HPV-DNA载量对应的OR值在CIN 2患者中分别为14.56、19.32、27.08,CIN 3患者中为14.87、20.06、28.54,宫颈癌患者中为2.53、7.09、18.33,均呈现出随着HPV-DNA载量级别增高风险增高的趋势。结论:采用HC2检测女性宫颈HPV-DNA载量可以对其宫颈病变程度进行判定,高HPV-DNA载量于CIN 2、CIN 3、宫颈癌病变严重程度密切相关。
OBJECTIVE: To investigate the correlation between HC2 detection of human papillomavirus DNA (HPV-DNA) load and the severity of cervical lesions in women. Methods: Seventy patients with chronic cervicitis, 40 patients with mild cervical dysplasia (CIN 1), 40 with moderate dysplasia (CIN 2), and severe atypical hyperplasia (CIN 3) were enrolled in our gynecology clinic. 40 cases of cervical cancer, 40 cases of cervical cancer were detected in patients with different types of cervical lesions and HPV-DNA load relationship. Results: The positive rate of HPV-DNA in cervicitis group was 51.43%, which was significantly lower than that in CIN 1 (80%), CIN 2 (95%), CIN 3 (95%) and cervical cancer group (82% 0.05). The positive rate of HPV-DNA in CIN 1 group was significantly lower than that in CIN 2 and CIN 3 groups (P <0.05). There was no significant correlation between HPVDNA grade and cervicitis in cervicitis patients (P> 0.05). The severity of cervical lesions in CIN 1, CIN 2, CIN 3 and cervical cancer patients was similar to that of HPV-DNA (P <0.05). The OR of HPV-DNA load was 14.56, 19.32, 27.08 respectively in CIN 2 patients, 14.87, 20.06, 28.54 in CIN 3 patients and 2.53 in cervical cancer patients, 7.09 and 18.33, all showed a trend of increasing risk with the increase of HPV-DNA loading level. Conclusion: The detection of cervical HPV-DNA load by HC2 can determine the degree of cervical lesions, high HPV-DNA load in CIN 2, CIN 3, cervical cancer severity is closely related.
