目的探讨鼠神经生长因子治疗新生儿胆红素脑损伤听力障碍的临床效果。方法选取2012年10月-2014年5月在湖南省儿童医院新生儿科住院、接受换血治疗并伴有重度听力障碍的重度高间接胆红素血症新生儿(均除外新生儿窒息、新生儿颅内出血、新生儿缺氧缺血性脑病等中枢神经受损和耳源性疾病导致的听力损伤),并完成随访~1岁的54例为研究对象;以2013年8月份后经家长同意接受鼠神经生长因子肌注治疗的患儿30例为观察组,以2013年8月前未应用鼠神经生长因子肌注的患儿24例为对照组,两组患儿除鼠神经生长因子肌注治疗上不同外其他治疗相同,随访1年比较两组患儿听力损伤的恢复情况。结果观察组患儿治愈10例,治愈率33.34%;好转11例,好转率36.66%;无效9例,无效率为30%。对照组患儿治愈2例,治愈率8.33%;好转5例,好转率20.83%;无效17例,无效率70.84%。观察组总有效率70.00%与对照组29.17%相比差异有统计学意义(P<0.01)。结论鼠神经生长因子对胆红素脑损伤听力障碍的患儿有一定的治疗效果。
Objective To investigate the clinical effect of murine nerve growth factor (NGF) on hearing loss of neonatal bilirubin injury. Methods From October 2012 to May 2014, newborns with severe hyperinfarction indirect hyperbilirubinaemia who were hospitalized in Department of Neonatology, Children’s Hospital of Hunan Province, who received blood transfusion and who had severe hearing impairment (except neonatal asphyxia, neonatal Internal bleeding, neonatal hypoxic ischemic encephalopathy and other central nervous system damage and ototoxicity caused by hearing loss), and completed the follow-up ~ 1 year old 54 cases as the research object; with the consent of the parents after August 2013 mice Thirty patients with intramuscular injection of nerve growth factor were treated as observation group. Twenty-four children without intramuscular injection of nerve growth factor (ADM) before August 2013 served as the control group. Two groups of children received intramuscular injection of nerve growth factor On the different outside the same treatment, followed up for 1 year compared the recovery of hearing loss in two groups of children. Results The observation group of children cured 10 cases, the cure rate was 33.34%; improved in 11 cases, the improvement rate of 36.66%; ineffective in 9 cases, the inefficiency of 30%. The control group of children cured in 2 cases, the cure rate was 8.33%; improved in 5 cases, the rate of improvement was 20.83%; 17 cases were ineffective, the inefficiency of 70.84%. The total effective rate was 70.00% in the observation group and 29.17% in the control group, the difference was statistically significant (P <0.01). Conclusion NGF has a certain therapeutic effect on children with bilirubin-induced hearing impairment.
