目的:探讨联合检测类风湿因子(RF)、人抗环胍氨酸肽抗体(CCP)和C-反应蛋白(CRP)在类风湿性关节炎(RA)中的诊断价值。方法:采用速率散射比浊法检测RF和CRP,酶联免疫吸附试验(ELISA)检测CCP。共检测395例RA疑似患者,确诊104例RA,作为本研究的A组;排除诊断291例,作为本研究的B组。结果:104例RA患者中RF、CCP、CRP阳性率分别为70.2%(73/104)、50%(52/104)、93.3%(97/104),291例非RA患者其阳性率分别为15.1%(44/291)、3.4%(10/291)、35.4%(103/291)。结论:CCP对RA高度特异,CRP对RA高度敏感,二者可与RF相互补充,三者联合检测能够提高RA诊断的灵敏度和特异性。
Objective: To explore the diagnostic value of combined detection of rheumatoid factor (RF), human anti cyclic cyclic arginine peptide antibody (CCP) and C - reactive protein (CRP) in rheumatoid arthritis (RA). Methods: RF and CRP were detected by rate nephelometry and CCP by enzyme-linked immunosorbent assay (ELISA). A total of 395 RA-suspected RA patients were diagnosed. A total of 104 RA patients were diagnosed as A group in this study, and 291 cases were excluded as B group in this study. Results: The positive rates of RF, CCP and CRP in 104 RA patients were 70.2% (73/104), 50% (52/104) and 93.3% (97/104), respectively. The positive rates of RF, CCP and CRP in 291 non RA patients were 15.1% (44/291), 3.4% (10/291), 35.4% (103/291). Conclusion: CCP is highly specific to RA, CRP is highly sensitive to RA, and both can be complemented with RF. Combined detection of these three markers can improve the sensitivity and specificity of RA diagnosis.
