二、兼咳喘症 40条曰:“伤寒表不解,心下有水气,干呕,发热而咳,或渴,或利,或噎,或小便不利,少腹满,或喘者,小青龙汤主之。”本条阐述的是,太阳伤寒表实证兼水饮内停而致咳喘症的证治。条中“伤寒表不解……发热而咳”论述其主证,为第一段;“或渴……或喘者”续议或然证,是第二段。第一段论述主证以“伤寒表不解,心下有水气”冒首,既是对全条病理机制的一个总括,又有喻明具体脉证的意义。《医宗金鉴》指出:“伤寒表不解,谓脉浮紧,头痛,身痛,发热,恶寒之证
