目的探讨激光辅助孵化(1aser-assisted hatching,LAH)对反复移植失败患者以及高龄患者冻融胚胎移植(frozen-thawed embryo transfer,FET)临床妊娠结局的影响。方法收集反复移植失败的患者98例,随机分为LAH 1组与对照组1,每组49例;高龄(≥38岁)FET患者90例,随机分为LAH 2组与对照组2,每组45例,LAH1组与LAH 2组患者均在移植前2小时进行LAH。结果 LAH1组的胚胎种植率、临床妊娠率和活产率均显著高于对照组1(28.84%vs11.5 3%;42.85%vs20.41%;36.7 3%vs16.32%);LAH 2组的胚胎种植率、临床妊娠率和活产率均高于对照组2(1 9.64%vs8.65%;42.22%vs20.00%;40.00%vs1 7.78%),差异有统计学意义(P<0.05)。结论 LAH可提高反复种植失败患者和高龄患者冻融胚胎移植的胚胎种植率、临床妊娠率和活产率,改善临床结局。
Objective To investigate the effect of laser-assisted hatching (LAH) on the clinical pregnancy outcomes of frozen-thawed embryo transfer (FET) patients undergoing repeated transplantation and elderly patients. Methods Ninety-eight patients with recurrent graft failure were randomly divided into LAH group 1 and control group 1, 49 cases in each group. 90 elderly patients (≥38 years old) with FET were randomly divided into LAH 2 group and control group 2, 45 cases, LAH1 group and LAH2 patients were 2 hours before transplantation for LAH. Results The embryo implantation rate, clinical pregnancy rate and live birth rate in LAH1 group were significantly higher than those in control group 1 (28.84% vs 11.5%, 42.85% vs 20.41%, 36.7% vs 16.32%, respectively) The implantation rate, clinical pregnancy rate and live birth rate of the control group were significantly higher than those of the control group 2 (9.62% vs 8.65% vs 42.22% vs 40.00% vs 7.78%, respectively) ). Conclusions LAH can improve the embryo implantation rate, clinical pregnancy rate and live birth rate of frozen-thawed embryo transfer patients and elderly patients with repeated implant failure and improve clinical outcome.
