目的探讨剪切波弹性成像(SWE)彩色图像特征评价乳腺纤维腺瘤的价值。方法回顾性分析经病理证实为乳腺纤维腺瘤患者98例(115个病灶)的SWE彩色图像特征,根据颜色六分法、弹性彩色图像分类法和弹性均质性评分评估病灶SWE彩色图像特征,测量病灶弹性模量值并分析其与三种评估方法的相关性。结果根据乳腺影像报告与数据系统(BI-RADS)诊断标准,共68个病灶需要活检,颜色六分法、弹性彩色图像分类法和弹性均质性结合BI-RADS活检率分别为20.9%、21.7%和36.5%,并且64.7%、63.2%和38.2%病灶避免了不必要的穿刺活检。三种彩色图像特征评估结果与最大弹性模量值、弹性标准差值均有显著相关性(P<0.01)。结论 SWE彩色图像特征评估作为常规超声的重要补充,可减少对于乳腺纤维腺瘤的活检率。
Objective To investigate the value of shear wave elastography (SWE) color image in the evaluation of breast fibroadenoma. Methods We retrospectively analyzed the SWE color images of 98 patients (115 lesions) with pathologically confirmed breast fibroadenoma. The characteristics of SWE color images were evaluated according to the color six-point method, flexible color image classification and elastic homogeneity score. The lesion elastic modulus values were measured and their correlations with the three assessment methods were analyzed. Results According to the BI-RADS diagnostic criteria, a total of 68 lesions were biopsied, the color six-point method, flexible color image classification and elastic homogeneity combined with BI-RADS biopsy were 20.9%, 21.7 % And 36.5% respectively, and 64.7%, 63.2% and 38.2% of the lesions avoided unnecessary biopsy. The results of three color images were significantly correlated with the maximum elastic modulus and elastic standard deviation (P <0.01). Conclusion The evaluation of SWE color image features as an important supplement to conventional ultrasound can reduce the biopsy rate of breast fibroadenoma.
