[目的]研究正常婴儿髋关节超声多种评价参数的相关性及多种测量方法从不同角度评价髋关节发育的临床意义。[方法]对105例婴儿[男56例,女49例,汉族72例,回族33例,月龄1~6个月,平均(2.6±1.3)个月]210个正常髋关节行超声检查,观察髋关节形态结构并测量不同参数(α角,β角,股骨头覆盖率,髋臼角,L值),比较不同参数相关性。[结果]1正常婴儿髋关节α角,β角,股骨头覆盖率测量值、髋臼角及L值测量值分别为(67.24±4.21)°,(62.92±6.40)°,(61.92±5.52)%,(23.04±2.56)°,(0.44±0.05);2正常男婴与女婴α角,β角,股骨头覆盖率,髋臼角及L值差异无统计学意义。回族与汉族婴儿正常髋关节α角,β角,股骨头覆盖率,髋臼角及L值差异无统计学意义;3α角与FHC呈正相关,与β角、髋臼角及L值呈负相关。[结论]Graf方法作为婴儿髋关节筛查的首选方法具有重要的临床意义;FHC、AI角、L值与α、β角具有相关性性,反映股骨头覆盖及骨性髋臼发育情况,对婴儿髋关节超声筛查具有借鉴意义。
[Objective] To study the correlation between multiple parameters of hip ultrasound in normal infants and the clinical significance of various measurement methods in evaluating hip development from different angles. [Methods] 210 normal hip radiographs were performed in 105 infants (56 male, 49 female, 72 Han, 33 Hui, 1 ~ 6 months old, mean 2.6 ± 1.3 months) Observe the morphology of hip joint and measure different parameters (α angle, β angle, femoral head coverage, acetabular angle, L value) and compare the correlation of different parameters. [Results] The measured values of α, β, femoral head coverage, acetabular angle and L value in normal infants were (67.24 ± 4.21), (62.92 ± 6.40) ° and (61.92 ± 5.52) %, (23.04 ± 2.56) ° and (0.44 ± 0.05) respectively; 2 There was no significant difference in the alpha, beta, femoral head coverage, acetabular angle and L between normal boys and infants. There were no significant differences in α, β, femoral head coverage, acetabular angle and L between the Hui and Han nationality infants. There was a positive correlation between 3α angle and FHC and a negative correlation with β angle, acetabular angle and L value . [Conclusion] The Graf method has important clinical significance as the preferred method of screening for hip joint in infants. The correlation between FHC, AI angle and L value and α, β angle reflects the coverage of femoral head and the development of bony acetabulum. Hip ultrasound ultrasound screening has reference.
