目的分析难治性妇产科大出血的特点及子宫动脉栓塞术对其治疗效果。方法 2013年4月到2014年1月选取96例大出血患者进行回顾性分析,将其分为两组,其中观察组52例患者,进行子宫动脉栓塞术治疗,对照组44例患者,给予传统子宫动脉结扎术,分析对比两组患者止血情况以及出血量、止血有效率、子宫保留率以及术后情况。结果观察组的手术时间[(41.8±7.1)min]、出血量[(983.8±225.7)ml]以及阴道出血持续时间[(12.1±2.3)min]显著低于对照组[(71.8±10.2)min、(1 483.8±246.7)ml、(17.4±2.1)min],差异均有统计学意义(P均<0.05);观察组止血有效率为61.54%(32/52),子宫保留率为63.46%(33/52),均显著高于对照组的38.64%(17/44)和40.91%(18/44),差异均有统计学意义(P均<0.05);两组患者术后均有不同程度发热,观察组术后日平均最高体温为(37.9±0.3)℃,对照组术后日平均最高体温为(37.8±0.5)℃,两组比较差异无统计学意义(P>0.05),两组下腹部不适、疼痛发生率比较差异无统计学意义(P均>0.05),两组均未发生其他并发症。结论子宫动脉栓塞术可以更有效的治疗难治性的妇产科大出血,挽救患者的生命并保证预后。
Objective To analyze the characteristics of intractable obstetrics and gynecology bleeding and the therapeutic effect of uterine arterial embolization. Methods From April 2013 to January 2014, 96 patients with major bleeding were selected and retrospectively analyzed. They were divided into two groups. Among them, 52 patients in the observation group were treated with uterine artery embolization. In the control group, 44 patients were given conventional uterus Arterial ligation, analysis and comparison of two groups of patients bleeding and bleeding, hemostatic efficiency, uterine retention rate and postoperative conditions. Results The operative time (41.8 ± 7.1) min, the amount of bleeding (983.8 ± 225.7) ml, and the duration of vaginal bleeding (12.1 ± 2.3) min in the observation group were significantly lower than those in the control group [(71.8 ± 10.2) min , (483.8 ± 246.7) ml, (17.4 ± 2.1) min respectively) (all P <0.05). The effective rate of hemostasis was 61.54% (32/52) in observation group and 63.46% (33/52) in the two groups were significantly higher than those in the control group (38.64%, 17/44) and 40.91% (18/44), respectively, with significant difference (all P <0.05) (37.9 ± 0.3) ℃ in the observation group, and (37.8 ± 0.5) ℃ in the control group after operation. There was no significant difference between the two groups (P> 0.05) Group lower abdominal discomfort, the incidence of pain was no significant difference (P all> 0.05), no other complications occurred in both groups. Conclusion Uterine artery embolization can be more effective treatment of intractable obstetric and gynecological bleeding, save the patient’s life and ensure the prognosis.
