目的了解四川省丹棱县血吸虫病流行动态和血吸虫病流行的相关因素,预测流行趋势,给国家制定血吸虫病防治策略和评价防治效果提供科学依据。方法在四川省丹棱县开展血吸虫病疫情监测,对2000-2014年监测数据资料进行统计分析。结果人群血吸虫血清学阳性率从2005年的22.09%下降到2014年的3.01%;血吸虫感染率从2000年的13.46%下降到2008年的0.81%,以后未粪检出阳性。男女感染率差异无统计学意义(P>0.05),男女患者感染度几何均数差异无统计学意义(P>0.05)。病例在年龄组中呈负偏态分布,以农民为主。农村人均纯收入与人群感染率呈高度负相关(P<0.05)。2000-2004年耕牛感染率分别为13.48%、37.36%、26.00%、10.53%和8.82%,以后未检出阳性。耕牛感染率与人群感染率呈高度正相关(P<0.05)。所有受检阳性人员和耕牛接受化疗。有螺面积从147 010 m~2下降到0 m~2后,回升到1 650 m~2;活螺框率从37.49%下降到0.72%,活螺密度从2.38只/0.1 m~2下降到0.02只/0.1 m~2。结论丹棱县血吸虫病疫情呈下降趋势,已处于低度流行状态。在经济发展的前提下,可考虑实施以控制传染源为主的策略。
Objective To understand the prevalence of schistosomiasis and the prevalence of schistosomiasis in Danling County, Sichuan Province, predict the epidemic trend, and provide a scientific basis for the prevention and control strategies of schistosomiasis and the evaluation of prevention and control. Methods To monitor the epidemic situation of schistosomiasis in Danling County, Sichuan Province, and to analyze the data of monitoring data from 2000 to 2014. Results The positive rate of schistosomiasis serogroups declined from 22.09% in 2005 to 3.01% in 2014; the infection rate of schistosomiasis dropped from 13.46% in 2000 to 0.81% There was no significant difference in infection rates between men and women (P> 0.05). There was no significant difference in the geometric mean of infection between men and women (P> 0.05). Cases in the age group was negative skewed distribution, mainly farmers. Rural per capita net income and population infection rate was highly negative correlation (P <0.05). The rates of cattle invaders from 2000 to 2004 were 13.48%, 37.36%, 26.00%, 10.53% and 8.82%, respectively. Cattle infection rate and population infection rate was highly positive (P <0.05). All tested positives and cattle received chemotherapy. After the snail area decreased from 147 010 m ~ 2 to 0 m ~ 2, it rose back to 1 650 m ~ 2, the live screw rate decreased from 37.49% to 0.72%, and the live screw density decreased from 2.38 /0.1 m ~ 2 to 0.02 / 0.1 m ~ 2. Conclusions The epidemic situation of schistosomiasis in Danling County is in a downward trend and has been in a low prevalence state. Under the premise of economic development, we can consider implementing a strategy that focuses on controlling sources of infection.
