目的探讨子宫内膜癌经阴道彩色多普勒超声(transvaginal colordoppler sonography,TVCDS)征象与微血管密度(microvessel density,MVD)的关系。方法32例经手术病理证实的子宫内膜癌患者术前应用TVCDS记录肿瘤大小、肌壁浸润深度、血流分级、阻力指数(RI),术后采用免疫组化SP法检测癌组织中MVD,与TVCDS征象进行对照分析。结果子宫内膜癌TVCDS图像上内膜厚度与MVD呈正相关(r=0.513,P=0.003);肌层浸润深度分组、血流分级之间MVD差异具有统计学意义(P<0.05);RI与MVD呈负相关(r=-0.743,P=0.000)。结论TVCDS征象与MVD相关,可间接评价肿瘤新生血管生成,为判断子宫内膜癌的发展、转移及预后提供客观依据。
Objective To investigate the relationship between transvaginal colordoppler sonography (TVCDS) signs and microvessel density (MVD) in endometrial carcinoma. Methods Thirty-two patients with endometrial carcinoma confirmed by surgery and pathology were enrolled in this study. Tumor size, depth of muscular wall invasion, blood flow classification and resistance index (RI) were recorded by TVCDS before operation. Immunohistochemical SP method was used to detect MVD, And TVCDS signs of controlled analysis. Results The thickness of intima of TVCDS in endometrial carcinoma was positively correlated with MVD (r = 0.513, P = 0.003). There was significant difference in MVD between muscular invasion depth group and blood flow classification (P <0.05) MVD was negatively correlated (r = -0.743, P = 0.000). Conclusions The signs of TVCDS are correlated with MVD, which can indirectly evaluate the tumor angiogenesis and provide an objective basis for judging the development, metastasis and prognosis of endometrial cancer.
