目的:探讨CT灌注成像(CTPI)联合CT血管造影(CTA)在椎基底动脉供血不足早期诊断中的作用。方法:随机选取临床诊断为椎-基底动脉供血不足(VBI)的患者45例作为观察组,选择同期45例健康者作为对照组。对两组病例均进行CTPI及CTA检查,并记录相关参数。结果:45例患者中CTPI异常者为36例,在CTPI各参数中,MTT阳性率最高,为100%,CBF次之,为72.2%,CBV最低,为27.8%;观察组的MTT、CBF差值的绝对值显著高于对照组(P<0.05),而CBV无差异(P>0.05);且CTA异常者CTPI阳性率显著高于CTA正常者(P<0.05)。结论:CTPI可早期反应脑灌注不足,CTA联合CTPI并对各参数进行分析可提高VBI的早期诊断水平。
Objective: To investigate the role of CT perfusion imaging (CTPI) combined with CT angiography (CTA) in the early diagnosis of vertebrobasilar insufficiency. Methods: Forty-five patients with clinically diagnosed as VBI were selected as the observation group and 45 healthy controls were selected as the control group. The two groups of patients were CTPI and CTA examination, and record the relevant parameters. Results: There were 36 cases with abnormal CTPI in 45 cases. The highest positive rate of MTT in CTPI was 100%, followed by CBF was 72.2% and CBV was the lowest (27.8%). MTT and CBF difference The absolute value of the CTPI was significantly higher than that of the control group (P <0.05), but no difference of CBV (P> 0.05). The positive rate of CTPI in patients with abnormal CTA was significantly higher than that of normal CTA (P <0.05). Conclusion: CTPI can early response cerebral perfusion insufficiency, CTA combined with CTPI and analysis of various parameters can improve the early diagnosis of VBI level.
