目的探讨双源螺旋CT冠状动脉成像对冠心病的诊断价值。方法对49例临床疑诊冠心病患者行双源螺旋CT冠状动脉成像和常规冠状动脉造影术,对显示冠状动脉狭窄进行定量评价。以冠状动脉造影结果作为诊断的金标准,计算出双源螺旋冠状动脉成像对冠状动脉狭窄的诊断准确性指标。结果 DSCT的灵敏度、特异度、PPV、NPV分别为90.0%、90.0%、94.0%、80.0%,诊断符合率为86.5%。DSCT和CAG对每支冠脉狭窄的检出差异经统计学处理,无统计学意义(P<0.05)。49例患者中,42例经CAG确诊存在明显冠状动脉狭窄,DSCT检出其中的41例,诊断准确度达97.6%。结论双源螺旋CT对冠状动脉疾病的诊断准确率接近选择性冠状动脉造影,是一种准确可靠的检查方法。
Objective To investigate the diagnostic value of double helical CT coronary angiography in coronary heart disease. Methods A total of 49 patients with clinically suspected coronary heart disease underwent double helical CT coronary angiography and conventional coronary angiography. The quantitative evaluation of coronary artery stenosis was performed. To coronary angiography as the gold standard for diagnosis, calculated dual-source helical coronary angiography of coronary artery stenosis diagnostic accuracy index. Results The sensitivity, specificity, PPV and NPV of DSCT were 90.0%, 90.0%, 94.0% and 80.0% respectively. The diagnostic coincidence rate was 86.5%. The difference of DSCT and CAG in detecting coronary stenosis was statistically significant (P <0.05). Of the 49 patients, 42 were diagnosed with coronary artery stenosis by CAG, 41 were detected by DSCT, and the diagnostic accuracy was 97.6%. Conclusion Dual-helical CT is an accurate and reliable method for the diagnosis of coronary artery disease, which is close to selective coronary angiography.
