目的 探讨CT测量脑中线移位预测脑疝的计分方法。方法 对临界脑疝组、脑疝组及对照组共458例脑外伤、脑出血病例,进行脑中线CT测量和统计学分析,寻找出脑中线各结构移位的脑疝危险系数,并将危险系数分级量化进行“脑疝预测计分”,对44例脑外伤、脑出血病例前瞻性计分评估。结果 脑中线各结构移位的脑疝危险系数是:中脑移位3mm,大脑镰移位6mm,三脑室移位9mm,透明隔移位12mm;“脑疝预测计分法”计分为6分时即为脑疝临界状态,分值增加危险性加大。结论 “脑疝预测计分法”是CT预测脑疝的一种快捷、简便、有效的方法。
Objective To investigate the scoring method of CT in predicting brain herniation by midline shift. Methods A total of 458 cases of traumatic brain injury and cerebral hemorrhage in the critical hernia group, the brain hernia group and the control group were subjected to midline CT scan and statistical analysis to find out the risk factors of herniation of each structure in the brain midline. Coefficient quantification of “brain herniation prediction score”, 44 cases of brain trauma, cerebral hemorrhage cases scoring prospective assessment. Results The risk factors of brain herniation were as follows: 3 mm in midbrain displacement, 6 mm in falx shift, 9 mm in third ventricle, and 12 mm in translocation. The score of “Prediction of cerebral hernia” was 6 Timeshare is the critical state of brain herniation, score increased risk increases. Conclusion “Brain herniation prediction scoring method” is a quick, simple and effective method for CT prediction of herniation.
