目的探讨Er:YAG激光辅助凝血在上前牙美学区种植修复中的临床应用及疗效。方法 2011年12月至2014年10月,本中心收治上前牙美学区域牙位缺失患者24例,埋植式植入种植体5个月后接受二期手术,随机分为激光照射组(12例)和非照射组(12例)。照射组采用Er:YAG激光辅助凝血,非照射组采用常规缝合止血。评价术后印模制作时间及制作终义齿时间,用PES(pink esthetic score)计分标准进行美学评价,并对患者满意度进行调查。结果24例均成功完成种植义齿修复,激光照射组制作终义齿时间为10.25±1.14天,非照射组制作终义齿时间为26.08±3.55天。差异具有统计学意义(P<0.05)。照射组PES得分最高14分,最低7分,中值为12.50;非照射组PES得分最高11分,最低7分,中值为7.00。差异具有统计学意义(P<0.05)。照射组83%患者对修复结果表示满意,17%患者表示基本满意。非照射组74%患者对修复结果表示满意,26%患者表示基本满意。结论激光辅助凝血促进种植体周围软组织修复,缩短临时及永久修复时间,美学区域行种植修复能够获得良好的临床效果。
Objective To investigate the clinical application and efficacy of Er: YAG laser-assisted coagulation in the implantation repair of the upper anterior dental aesthetic area. Methods From December 2011 to October 2014, 24 patients with missing teeth in the upper anterior aesthetic area were treated. The implants were implanted 5 months later and were randomly divided into laser irradiation group (12 Cases) and non-irradiation group (12 cases). Irradiated with Er: YAG laser-assisted coagulation group, non-irradiated group using conventional suture hemostasis. Evaluate postoperative impression making time and final denture time, aesthetically evaluate with PES (pink esthetic score) score, and investigate patient satisfaction. Results All the 24 cases were successful in implant denture. The final denture time in laser irradiation group was 10.25 ± 1.14 days and the final denture time in non-irradiation group was 26.08 ± 3.55 days. The difference was statistically significant (P <0.05). The PES score of the irradiation group was 14, the lowest was 7, and the median was 12.50. The non-irradiated group had the highest PES score of 11 and the lowest score of 7, with a median of 7.00. The difference was statistically significant (P <0.05). 83% of the patients in the irradiation group were satisfied with the results of the repair, and 17% of the patients were basically satisfied. 74% of patients in the non-irradiated group were satisfied with the results of the repair, and 26% of the patients showed satisfactory satisfaction. Conclusion Laser-assisted coagulation promotes soft tissue restoration around the implant, shortening the time of temporary and permanent repair, and the aesthetic repair of the area can achieve good clinical results.
