本文报告５１例脑弥漫性轴索损伤（ＤＡＩ），约占同期重型颅脑损伤的１０３％。将５１例病人根据其临床表现及ＣＴ特征不同分成四个类型：皮质下型（Ⅰ型）、中央型（Ⅱ型）、脑干型（Ⅲ型）及混合型（Ⅳ型、Ⅳ型又分成Ⅳａ及Ⅳｂ两个亚型）。全组死亡２６例，死亡率５０１％。ＤＡＩ各型中以Ⅰ型预后为好，Ⅲ型及Ⅳｂ型预后最差。此分型方案能直接估计ＤＡＩ病情严重程度及预后。
This article reports 51 cases of diffuse axonal injury (DAI), accounting for about 10 3% of severe craniocerebral injury in the same period. Fifty-one patients were divided into four groups based on their clinical manifestations and CT features: subcortical (type I), central (type II), brainstem (type III) and mixed (type IV and IV) IVa and IVb two subtypes). The whole group died in 26 cases, the mortality rate was 501%. In all types of DAI, type Ⅰ prognosis is better, and type Ⅲ and Ⅳb have the worst prognosis. This classification scheme can directly estimate the severity and prognosis of DAI.
