目的:探讨CA125和超声联合应用在卵巢癌术前诊断中的价值。方法:对148例卵巢肿瘤患者(其中良性100例、恶性48例)于术前1周内,经阴道二维超声观察肿块形态特征、彩色多谱勒观察肿瘤内部及表面的血管分布情况,并计算出阻力指数(RJ),放射免疫分析法测定血清CA125值,选定一界点值,对卵巢癌术前进行初步诊断,并与术后病理结果对照,观察其诊断价值。结果:三种方法联合应用在术前诊断卵巢癌中的敏感性、特异性、阳性预测值及阴性预测值分别为97.92%、97%、94%及98.98%。结论:CA125和超声联合应用提高了术前诊断卵巢癌的准确性,其诊断价值明显优于一种方法单独应用或其他方法联合应用。
Objective: To investigate the value of combined application of CA125 and ultrasound in preoperative diagnosis of ovarian cancer. Methods: 148 cases of ovarian tumors (including 100 cases of benign and malignant 48 cases) within 1 week before operation, the morphological features of the tumor were observed by two-dimensional vaginal ultrasound, and the distribution of blood vessels inside and outside the tumor was observed by color Doppler. The resistance index (RJ) was calculated. The value of serum CA125 was determined by radioimmunoassay. The value of a threshold was selected to preliminarily diagnose ovarian cancer. The diagnostic value was compared with the postoperative pathological results. Results: The sensitivity, specificity, positive predictive value and negative predictive value of the three methods in preoperative diagnosis of ovarian cancer were 97.92%, 97%, 94% and 98.98% respectively. Conclusion: The combination of CA125 and ultrasound improves the accuracy of preoperative diagnosis of ovarian cancer. The diagnostic value of CA125 is superior to that of one method alone or combined with other methods.
