目的:探讨先天性心脏病(先心病)合并肺动脉高压患儿围术期血浆内皮素浓度动态变化规律及血浆内皮素浓度与肺动脉压力关系。方法:用放射免疫法对20例先心病患儿围术期血浆内皮素浓度进行测定,用直线相关法分析血浆内皮素浓度与肺动脉压力之间关系。结果:肺动脉高压组术前血浆内皮素浓度明显高于对照组,且与肺动脉压呈显著正相关,r=0.97、P<0.01。体外循环期间,血浆内皮素浓度逐渐升高,体外循环结束时达高峰,术后逐渐下降,但肺动脉高压组术后3天血浆内皮素浓度仍高于术前。结论:血浆内皮素在肺动脉高压形成、发展过程中起一定作用,心脏直视手术过程中血浆内皮素浓度升高与体外循环密切相关。
Objective: To investigate the dynamic changes of perioperative plasma endothelin concentration and plasma endothelin concentration and pulmonary artery pressure in children with congenital heart disease (CHD) complicated with pulmonary hypertension. Methods: Perioperative plasma endothelin levels were measured in 20 children with congenital heart disease by radioimmunoassay. The correlation between plasma endothelin concentration and pulmonary artery pressure was analyzed by linear correlation method. Results: Preoperative plasma endothelin concentration in pulmonary hypertension group was significantly higher than that in control group, and was positively correlated with pulmonary artery pressure (r = 0.97, P <0.01). During cardiopulmonary bypass, plasma endothelin concentration gradually increased, reached the peak at the end of cardiopulmonary bypass, and then decreased gradually. However, the plasma endothelin concentration in pulmonary hypertension group was still higher than that before operation three days after operation. CONCLUSION: Plasma endothelin plays a role in the formation and development of pulmonary hypertension. The increase of endothelin concentration during open heart surgery is closely related to cardiopulmonary bypass.
