目的探讨潘生丁负荷/再分布201铊-SPECT心肌灌注显像评价飞行人员隐匿性冠心病预后的可行性。方法入选1998至2005年期间94例无任何症状、但运动平板试验反复阳性的飞行人员,根据其潘生丁负荷/再分布心肌201T1-SPECT显像结果分为检查结果阳性及阴性组,随访2年。记录该人群不良心脏事件发生情况,比较两组间的心血管事件发生率(MACE,major adverse cardiac event)及病死率。结果平均随访4.5年,201T1-SPECT心肌显像阳性组与阴性组的MACE分别为30%与7%,差异有统计学意义(P<0.01)。心肌显像缺血累及2个以上节段组MACE发生率明显高于单个节段受累组、2个节段受累组及心肌显像结果阴性组(P<0.05)。结论潘生丁负荷心肌201T1-SPECT显像有助于隐匿性冠心病预后判断,从而指导临床早期干预治疗。
Objective To investigate the feasibility of dipyridamole loaded / redistributive 201 thallium-SPECT myocardial perfusion imaging in evaluating the prognosis of occult coronary artery disease in pilots. Methods Ninety-four patients without any symptom between 1998 and 2005 were selected. However, the repeat positive motor test was divided into two groups according to their dipyridamole load / redistribution myocardial 201T1-SPECT imaging results. The incidence of adverse cardiac events in this population was recorded and the incidence of MACE (major adverse cardiac event) and mortality was compared between the two groups. Results The average follow-up of 4.5 years showed that the MACE of positive group and negative group of 201T1-SPECT myocardial perfusion imaging were 30% and 7%, respectively, with statistical significance (P <0.01). The incidence of MACE in myocardial ischemia in more than two segments was significantly higher than that in single segment, two segments and myocardial imaging (P <0.05). Conclusion Dipyridamole myocardial infarction 201T1-SPECT imaging contribute to the prognosis of occult coronary heart disease to guide the clinical early intervention.
