目的探讨低剂量多层螺旋CT(MSCT)筛查早期肺癌在临床工作中的可行性。方法常年吸烟或患有慢性阻塞性肺疾病的体检者210名,年龄平均48(43~57)岁(单位体检和自行主张要求检查者)。采用Geliqhtspeed ultra(MSCT)扫描机。参数为:螺旋扫描方式,120kV,50mA,螺距1.35∶1,27mm/r,重建层厚2.5mm,间隔2.5mm,肺重建算法,扫描时间12~15s,肺窗-600HU,窗宽1500HU。以多平面重建(MPR)电影方式观察,图象存储方式为DVD光盘。检查费用为200元人民币。结果检出非钙化结节32例,2例为1期病变。结论低剂量MSCT筛查早期肺癌,无论在安全剂量上、检出质量上,还是在成本和医者费用上都是可行的。
Objective To investigate the feasibility of low-dose multi-slice spiral CT (MSCT) screening of early lung cancer in clinical work. Methods Totally 210 subjects with smoking or chronic obstructive pulmonary disease were aged 48 to 43 years old (physical examination and self-claimed examination). A Geliqhtspeed ultra (MSCT) scanner is used. Parameters: spiral scan mode, 120kV, 50mA, pitch 1.35:1,27mm / r, reconstruction thickness 2.5mm, interval 2.5mm, lung reconstruction algorithm, scan time 12 ~ 15s, lung window -600HU, window width 1500HU. To multi-plane reconstruction (MPR) film mode of observation, the image is stored as a DVD disc. Check the cost of 200 yuan. Results Non-calcified nodules were detected in 32 cases and in 2 cases as stage 1 lesions. Conclusion Low-dose MSCT screening of early-stage lung cancer is feasible both in terms of safe dose, quality of detection, and cost and cost of the physician.
