目的:研究功能性便秘(FC)患者皮层-腰骶-肛门直肠神经通路的功能状态,探索FC潜在的神经机制。方法:FC组患者60例,男28例,女32例,平均年龄(49.44±15.38)岁;正常对照组25例,男11例,女14例,平均年龄(24.67±7.14)岁。通过经颅磁刺激皮层(TMS)和磁刺激腰脊神经根直肠内记录运动诱发电位(MEP)的检测方法观察神经反应电位的改变。结果:①TMS引出MEP结果:与正常对照组比较,FC组MEP波幅显著降低(P<0.05),波潜伏期和峰间期无显著变化;FC组波形分化不良,重复性差;FC组和正常对照组双侧MEP各参数比较均无显著差异;②磁刺激腰脊神经根MEP结果:FC组波潜伏期延长,波幅下降,差异显著(P<0.05)。结论:皮层-腰骶-肛门直肠神经通路的传导障碍可能是FC患者发病的潜在神经生理学机制。
Objective: To investigate the functional status of cortical-lumbosacral-anorectal nerve pathway in patients with functional constipation (FC) and to explore the underlying neural mechanism of FC. Methods: There were 60 patients in the FC group, including 28 males and 32 females, with an average age of 49.44 ± 15.38 years old. There were 25 normal controls, 11 males and 14 females with an average age of (24.67 ± 7.14) years. Neuroreactive potential changes were observed by transcranial magnetic stimulation cortex (TMS) and magnetic stimulation of lumbar spine nerve root rectum recording motor evoked potentials (MEP). Results: (1) The MEP was induced by TMS: The amplitude of MEP in FC group was significantly lower than that in normal control group (P <0.05), and there was no significant difference in wave latency and peak interval between FC group and normal group There was no significant difference in the parameters of bilateral MEP between the two groups. (2) MEP of lumbar nerve root was stimulated by magnetic stimulation: the latency of wave in FC group was prolonged and the amplitude of wave was decreased with significant difference (P <0.05). CONCLUSIONS: The conduction disturbance of the cortical-lumbosacral-anorectal pathway may be a potential neurophysiological mechanism of the onset of FC.
